New Publication: ESH Manual, Chapter 28, Incident Investigation

This is a major substantive revision to meet the requirements of the SLAC-OIO-2006-#D7-18 corrective action (CATS ID 3934). Changes in process and policy are as follows:

  • The investigation process for incidents involving an injury now includes a review by the new Incident Review and Assistance Team (IRAT), comprising the workers' compensation administrator, occupational health physician, incident investigation program manager, and a DOE SLAC Site Office (DOE SSO) representative. The team will meet weekly to review such investigations for completeness and to ensure that causes are adequately identified and that appropriate corrective actions are taken. The investigation process is diagramed in Incident Investigation: Occupational Injury Investigation Flow Chart.
  • The investigation process for incidents involving a government-owned vehicle now includes a review by the new Vehicle Incident Review and Assistance Team (VIRAT), comprising the SLAC Site Security manager, incident investigation program manager, a Fleet Services representative, and a DOE SSO representative. The team will review each vehicle incident to determine cause and chargeability. The investigation process is diagramed in Incident Investigation: Government-owned Vehicle Incident Investigation Flow Chart.
  • Notification requirements have been updated to reflect current policy: call 911 for life-threatening emergencies; call supervisor and/or extension 5555 in all cases. Proper notification initiates the investigation process, as described in procedures for each incident type.
  • The incident investigation form completed by an injured employee has been revised and is now called the Employee First Report of Injury (SU-17A). As before, it is only available from the SLAC Medical Department. This form was previously part A of the SU-17 form.
  • The investigation form completed by the supervisor responsible for the injured party or the area where an incident occurred is now called incident investigation form (SU-17B). The new form combines the original incident investigation form and part B of the SU-17. It will be available online.
  • The incident investigation form (SU-17B) must now be completed by the supervisor within three work days of knowledge of the incident; previously the requirement was that it had to be completed within five days.
  • A new summary of requirements in table form provides an overview of the different type of incidents and their investigation requirements (Incident Investigation: Requirements Summary).

This revision replaces the existing Chapter 28, "Incident Investigation", published in August 2006.
