New Procedures for the Purchase of Prescription Safety Glasses (Effective 04/16/07)

SLAC will reimburse up to $125 toward the cost of single vision, up to $150 for bifocal, and up to $175 for trifocal/progressive prescription safety glasses for employees in jobs that require eye protection.

The reimbursement cost will be charged to the employee's department/group.

PRIOR to the purchase of the prescription safety glasses, the employee must complete a Prescription Safety Glasses Approval Form to be reimbursed. The form must be approved and dated by a manager or supervisor. The completed approval form must be attached to a completed petty cash form with the original receipt and presented to the Petty Cash Office for reimbursement.

Department heads, supervisors, and managers decide who within their department/group requires prescription safety glasses. The ES&H Chemical & General Safety Department and SLAC's Medical Department are available to assist in defining eye-hazard operations. Generally, Occupational Safety and Health Administration (OSHA) regulations require eye protection—safety goggles or spectacles—"where there is a reasonable probability of injury that can be prevented by such equipment."

Employees MUST purchase their prescription safety glasses from SLAC's designated vendor:

All prescription safety glasses purchased from Stanford Optical are ANSI Z87.1 compliant.

Please call Stanford Optical to make an appointment. You must bring a Dispenser Optical Order Form to the scheduled appointment. The Dispenser Optical Order Form is available at the Petty Cash Office, Human Resources, or online at:

You will need to know your SLAC System ID number which is located under your name in the SLAC Directory. Use the SLAC System ID number as the Rx number on the form.

Bring your current optical prescription to the initial visit or the prescription can be faxed to 650-736-0575. IMPORTANT: Expired prescriptions are not accepted. The cost for an eye examination is not reimbursed by SLAC.

In some cases, there may be a safety reason for purchasing two pair of prescription safety glasses. If so, the employee’s supervisor must submit a brief memo describing the safety issue involved to Sharon Haynes, Human Resources: shaynes@SLAC.Stanford.edu. If the purchase of two pair is approved, a second Prescription Safety Glasses Approval Form must be completed for the second pair. For reimbursement, the completed forms must be attached to a completed petty cash slip along with the receipts and a copy of the memo marked "Approved."

Please note that SLAC's policy does not reimburse the cost for computer glasses or for sun glasses.
